Gavin Newsom spars with&nbsp;President Donald&nbsp;Trump and courts national attention&nbsp;for&nbsp;a potential presidential bid,&nbsp;at home&nbsp;he\u2019s&nbsp;catching&nbsp;flak&nbsp;from the left and the right&nbsp;on&nbsp;health care.&nbsp;<\/p>\n\n\n\n<p>The\u00a0California\u00a0Democrat\u00a0came into office promising to fight for\u00a0\u201c<a href=\"https:\/\/www.gov.ca.gov\/2019\/01\/07\/newsom-inaugural-address\/\" target=\"_blank\" rel=\"noreferrer noopener\">guaranteed\u00a0health care for all<\/a>,\u201d\u00a0and he came close to achieving it. Really close. But as it turns out,\u00a0that\u2019s\u00a0easier said than done when\u00a0you\u2019re\u00a0juggling\u00a0<a href=\"https:\/\/spectrumlocalnews.com\/nc\/charlotte\/politics\/2026\/01\/09\/newsom-faces-multibillion-dollar-budget-shortfall\" target=\"_blank\" rel=\"noreferrer noopener\">chronic budget\u00a0deficits<\/a>,\u00a0<a href=\"https:\/\/www.sfchronicle.com\/health\/article\/california-medicaid-medi-cal-borrowing-20220421.php\" target=\"_blank\" rel=\"noreferrer noopener\">rising health care costs<\/a>,\u00a0and\u00a0<a href=\"https:\/\/kffhealthnews.org\/news\/article\/state-budget-fallout-trump-health-funding-cuts-obbba\/\" target=\"_blank\" rel=\"noreferrer noopener\">shrinking federal support<\/a>.\u00a0<\/p>\n\n\n\n<p>Now&nbsp;he\u2019s&nbsp;walking the fine line between&nbsp;keeping&nbsp;his&nbsp;<a href=\"https:\/\/www.politico.com\/story\/2018\/11\/15\/california-governor-single-payer-healthcare-955627\" target=\"_blank\" rel=\"noreferrer noopener\">early promises to progressives<\/a>&nbsp;and being tarred as&nbsp;a reckless&nbsp;state&nbsp;executive who has stretched California\u2019s&nbsp;spending&nbsp;<a href=\"https:\/\/www.facebook.com\/reel\/1345401989882663\" target=\"_blank\" rel=\"noreferrer noopener\">beyond its means<\/a>.&nbsp;<\/p>\n\n\n\n<p>After&nbsp;years of political&nbsp;infighting,&nbsp;Newsom&nbsp;and the Democratic-controlled&nbsp;legislature&nbsp;in 2024&nbsp;broadened&nbsp;California\u2019s Medicaid program, Medi-Cal, to&nbsp;<a href=\"https:\/\/mcweb.apps.prd.cammis.medi-cal.ca.gov\/news\/32424\" target=\"_blank\" rel=\"noreferrer noopener\">all income-eligible&nbsp;children and adults<\/a>&nbsp;regardless of immigration status.&nbsp;<\/p>\n\n\n\n<p>Now,&nbsp;he\u2019s&nbsp;rolling&nbsp;back those expansions in the name of \u201cfiscal prudence.\u201d&nbsp;<\/p>\n\n\n<div\n\tclass=\"wp-block block--newsletter  \"\n\tdata-type=\"kaiser-health-news\/newsletter\"\n\tdata-align=\"center\"\n\tstyle=\"\"\n>\n\t<h4 class=\"newsletter__title\">\n\t\t<a href=\"\/email\/\">\n\t\t\tEmail Sign-Up\t\t<\/a>\n\t<\/h4>\n\t<p class=\"newsletter__description\">\n\t\tSubscribe to KFF Health News&#039; free weekly newsletter, &quot;The Week in Brief.&quot;\t<\/p>\n\t\n<form\n\taction=\"\/email-signup\/\"\n\tclass=\"kaiser-hubspot-mini-signup-form\"\n\tmethod=\"post\"\n>\n\t<input type=\"hidden\" id=\"kaiser_hubspot_nonce\" name=\"kaiser_hubspot_nonce\" value=\"dc9818bd64\" \/><input type=\"hidden\" name=\"_wp_http_referer\" value=\"\/wp-json\/wp\/v2\/article\/2152194\" \/>\t\t\t\t<label\n\t\tclass=\"kaiser-hubspot-mini-signup-form__label--email\"\n\t\tfor=\"kaiser-hubspot-mini-signup-form-email\"\n\t>\n\t\t<span class=\"screen-reader-text\">\n\t\t\tYour Email Address\t\t<\/span>\n\t\t<input\n\t\t\tautocomplete=\"email\"\n\t\t\tclass=\"kaiser-hubspot-mini-signup-form__input--email\"\n\t\t\tid=\"kaiser-hubspot-mini-signup-form-email\"\n\t\t\tname=\"kaiser_hubspot_email\"\n\t\t\ttype=\"email\"\n\t\t\t\t\t\t\tplaceholder=\"Your email address\"\n\t\t\t\t\t\/>\n\t<\/label>\n\t<button class=\"kaiser-hubspot-mini-signup-form__submit\">\n\t\tSign Up\t<\/button>\n<\/form>\n<\/div>\n\n\n\n<p>This year, California&nbsp;<a href=\"https:\/\/kffhealthnews.org\/news\/article\/california-newsom-budget-immigrants-medicaid-coverage-medi-cal\/\" target=\"_blank\" rel=\"noreferrer noopener\">froze&nbsp;Medi-Cal&nbsp;enrollment<\/a>&nbsp;for&nbsp;most&nbsp;adults&nbsp;without legal status, just&nbsp;two years after&nbsp;<a href=\"https:\/\/www.dhcs.ca.gov\/services\/medi-cal\/eligibility\/Pages\/Adult-Expansion.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">making them&nbsp;all&nbsp;eligible<\/a>. On July 1, immigrants not eligible for&nbsp;federal Medicaid&nbsp;\u2014&nbsp;both legal residents and those without authorization&nbsp;\u2014&nbsp;will lose access to&nbsp;state&nbsp;dental coverage.&nbsp;Next year,&nbsp;they\u2019ll&nbsp;have to start paying monthly premiums.&nbsp;<\/p>\n\n\n\n<p>Last month, Newsom proposed&nbsp;letting&nbsp;roughly 200,000&nbsp;legal immigrants&nbsp;\u2014&nbsp;asylees, refugees,&nbsp;and others&nbsp;\u2014&nbsp;get&nbsp;cut&nbsp;off from Medi-Cal after Sept. 30, when the federal government will stop paying for them.&nbsp;<\/p>\n\n\n\n<p>Advocates are livid.&nbsp;<\/p>\n\n\n\n<p>Progressives\u00a0say\u00a0Newsom\u2019s political ambitions\u00a0\u2014\u00a0and perceived need to distance himself from the\u00a0polarized\u00a0topic of immigrant health care\u00a0\u2014\u00a0go against\u00a0his early\u00a0pledges.\u00a0<\/p>\n\n\n\n<p>\u201cYou\u2019re clouded by what Arkansas is going to think, or Tennessee is going to think, when&nbsp;what California thinks is something completely different,\u201d said California&nbsp;state Sen. Caroline Menjivar, chair&nbsp;of the&nbsp;budget subcommittee on health&nbsp;and human services.&nbsp;<\/p>\n\n\n\n<p>Meanwhile, Republicans and fiscal hawks&nbsp;have&nbsp;painted&nbsp;Newsom as a&nbsp;<a href=\"https:\/\/www.reformcalifornia.org\/news\/reform-california-warns-of-massive-tax-increases-in-governor-newsoms-2023-budget-as-state-faces-fiscal-crisis\" target=\"_blank\" rel=\"noreferrer noopener\">tax-and-spend<\/a>&nbsp;Democrat prioritizing&nbsp;use of&nbsp;limited state funds on free health care for noncitizens.&nbsp;And Newsom has taken hits from&nbsp;the&nbsp;Trump administration accusing California of&nbsp;\u201c<a href=\"https:\/\/www.cms.gov\/newsroom\/press-releases\/cms-moves-shut-down-medicaid-loophole-protects-vulnerable-americans-saves-billions\" target=\"_blank\" rel=\"noreferrer noopener\">gaming&nbsp;the system<\/a>\u201d&nbsp;to use federal funds for immigrant health services.&nbsp;<\/p>\n\n\n\n<p>He\u2019s&nbsp;not the only governor grappling with this dilemma.&nbsp;And all 50 states,&nbsp;which&nbsp;are&nbsp;currently&nbsp;required to&nbsp;provide health coverage to&nbsp;refugees, asylees,&nbsp;and&nbsp;others,&nbsp;will have to&nbsp;decide whether to&nbsp;backfill that coverage for some 1.4 million legal immigrants starting Oct. 1, when&nbsp;<a href=\"https:\/\/www.kff.org\/immigrant-health\/1-4-million-lawfully-present-immigrants-are-expected-to-lose-health-coverage-due-to-the-2025-tax-and-budget-law\/\" target=\"_blank\" rel=\"noreferrer noopener\">a provision<\/a>&nbsp;of&nbsp;the One Big Beautiful Bill Act&nbsp;kicks in&nbsp;and leaves states without federal reimbursement for their care.<\/p>\n\n\n\n<div class=\"is-style-classic
