A toddler waved as <a href=\"https:\/\/www.mchp.care\/provider\/patricia-hall%2C-do\/chief-medical-officer\"><\/a>Patricia Hall walked past him, a stethoscope draped over her neck. The family physician waved and smiled back.<\/p>\n\n\n\n<p>But in the privacy of a conference room, her mood shifted. She is often bogged down with paperwork and can\u2019t get patients timely appointments with specialists. She also fears that a health care <a href=\"https:\/\/mahec.net\/rhi\/wnc-rural-health\">worker shortage<\/a> affecting her clinic \u2014 and many others across the nation \u2014 will only get worse.<\/p>\n\n\n\n<p>Hall\u2019s clinic, the Celo Health Center, is one of seven locations that make up the <a href=\"https:\/\/www.mchp.care\/\">Mountain Community Health Partnership<\/a>, which was supposed to receive up to $10 million over the next decade through <a href=\"https:\/\/www.cms.gov\/priorities\/innovation\/innovation-models\/making-care-primary\">Making Care Primary<\/a>, a federal program to improve primary care, <a href=\"https:\/\/www.cms.gov\/newsroom\/press-releases\/cms-announces-multi-state-initiative-strengthen-primary-care\">especially in rural areas<\/a>, by providing payments for physicians to address patient needs. Her organization planned to use the money to hire staff, build better connections with local specialists, and buy more vehicles to shuttle patients to appointments, according to its CEO, <a href=\"https:\/\/www.linkedin.com\/in\/tim-evans-msw-79292458\/\"><\/a><a href=\"https:\/\/www.mchp.care\/administration\">Tim Evans<\/a>.<\/p>\n\n\n\n<p>Then, in March, clinic administrators received an email during the Department of Government Efficiency\u2019s whirlwind of federal cuts: The Centers for Medicare &amp; Medicaid Services would soon shutter Making Care Primary, a year into what was supposed to be a 10-year program. Nearly 700 practices in eight states <a href=\"https:\/\/www.cms.gov\/priorities\/innovation\/where-innovation-happening#model=making-care-primary-mcp-model\">enrolled in Making Care Primary<\/a>. North Carolina had 23 clinics and centers in the program, the most of any state, followed by Washington, New Mexico, and New York. Doctors who had signed up for it said they were stunned.<\/p>\n\n\n\n<p>\u201cI&#8217;m angry, but more than that, I am so very sad,\u201d <a href=\"https:\/\/www.mchp.care\/provider\/patricia-hall%2C-do\/chief-medical-officer\">Hall said<\/a>. \u201cIt&#8217;s heartbreaking \u2014 to have an already inadequate health care system be made even more inadequate, to throw away opportunities to improve, even a little.\u201d<\/p>\n\n\n\n<figure class=\"wp-block-image alignwide size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"3500\" height=\"2333\" src=\"https:\/\/kffhealthnews.org\/wp-content\/uploads\/sites\/2\/2026\/02\/Making-Care-Primary-06-resized.jpg\" alt=\"A sign in a wooded area. The federal government promised up to $10 million in funding to the operation but revoked it in 2025.<span class=\"photo-credit\">(Andrew Jones\/KFF Health News)<\/span><\/figcaption><\/figure>\n\n\n\n<p>Now, CMS\u2019 Innovation Center, which created Making Care Primary, is set to launch a new 10-year program to boost primary care called the <a href=\"https:\/\/www.cms.gov\/priorities\/innovation\/innovation-models\/lead\">Long-term Enhanced ACO Design<\/a>, known by its acronym LEAD. In that program, funding will be sent to health care organizations, often managed by companies, instead of directly to primary care doctors and clinics.<\/p>\n\n\n\n<p>CMS wouldn\u2019t say how much the programs cost. But it maintained that eliminating Making Care Primary reduced spending <a href=\"https:\/\/www.cms.gov\/newsroom\/fact-sheets\/cms-innovation-center-announces-model-portfolio-changes-better-protect-taxpayers-and-help-americans\">without sacrificing its mission<\/a> to improve quality of care. Nixing Making Care Primary and three other programs at the agency \u2014 including another focused on primary care \u2014 would <a href=\"https:\/\/www.cms.gov\/newsroom\/fact-sheets\/cms-innovation-center-announces-model-portfolio-changes-better-protect-taxpayers-and-help-americans#:~:text=Guiding,early%2E\">save taxpayers $750 million<\/a>, CMS said at the time.<\/p>\n\n\n\n<p>Making Care Primary \u201cwas not on track to meet its intended savings goal,\u201d CMS spokesperson Alexx Pons said. Innovation Center models <a href=\"https:\/\/www.cms.gov\/priorities\/innovation\/about#:~:text=Center-,The%20CMS,CHIP%29%20programs%2E\">are meant<\/a> to save money over time, in part by improving patients\u2019 access to health care and helping them avoid costly hospital visits. Practices that signed up for Making Care Primary and want to join LEAD will have to apply for the program beginning in March.<\/p>\n\n\n\n<p>Yet Making Care Primary\u2019s elimination has created skepticism among doctors. In 2023, more than 100 million people in the U.S. <a href=\"https:\/\/www.nachc.org\/wp-content\/uploads\/2023\/06\/Closing-the-Primary-Care-Gap_Full-Report_2023_digital-final.pdf\">had no access<\/a> to a primary care doctor in their area, the National Association of Community Health Centers reported. Some states, such as Colorado, have passed legislation <a href=\"https:\/\/chir.georgetown.edu\/states-increasingly-use-power-over-commercial-health-insurance-to-boost-primary-care-investment\/\">to ensure<\/a> more primary care funding at the state level.<\/p>\n\n\n\n<p>The health care shortage where Hall works in western North Carolina is <a href=\"https:\/\/www.ncdhhs.gov\/nc-dhhs-orh-hpsa-one-pager\/open\">so severe<\/a>, she said she often makes personal calls to doctors to schedule appointments.<\/p>\n\n\n\n<p>Hall said one of her patients is an uninsured woman who has been waiting six months for a colonoscopy. The patient has severe anemia and possibly gastrointestinal bleeding. Hall has been trying to get the patient somewhere that offers free or discounted care.<\/p>\n\n\n\n<p>The additional funding through Making Care Primary would have allowed the network of clinics to improve its communication systems with specialists. That included patients\u2019 transportation to health care facilities, food vouchers, moving expenses, or help with utility bills.<\/p>\n\n\n\n<p>Spokespeople for health departments in the states that signed up for Making Care Primary said some doctors have since lost trust in federal support.<\/p>\n\n\n\n<p>\u201cIt can be difficult for providers, especially smaller community base practitioners, to pool resources to invest in non-clinical development,\u201d said Cadence Acquaviva, a spokesperson for New York\u2019s health agency. \u201cAn abrupt change can damage the confidence in future programs\u2019 sustainability.\u201d<\/p>\n\n\n\n<p><strong>An Uncertain Future<\/strong><\/p>\n\n\n\n<p>The CMS Innovation Center tests health care models to find those that improve care and lower costs for the health system and patients. The center created Making Care Primary after testing it as one of those models. It\u2019s unclear whether the models have ultimately saved money; a 2023 Congressional Budget Office report <a href=\"https:\/\/www.cbo.gov\/publication\/59274\">criticized the Innovation Center<\/a> for increasing spending by nearly double the amount the CBO projected the center would save in taxpayer money over 10 years.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.linkedin.com\/in\/liz-fowler-healthcare\/\">Elizabeth Fowler<\/a>, who directed the center under President Joe Biden, said Making Care Primary ended so early that it could not have produced any meaningful data on whether the patients it served became healthier.<\/p>\n\n\n\n<p>She acknowledged that the program didn\u2019t draw in as many clinics as she hoped and lagged in enrollment. But if it were her decision to make, \u201cI would have said, \u2018It\u2019s not enough time,\u2019\u201d Fowler said. \u201cIt takes more than a year to get the numbers.\u201d<\/p>\n\n\n\n<p>Hathaway said his Mountain Area Health Education Center signed on to Making Care Primary believing it could change the profession: It promised to ensure practices received immediate, consistent federal money to enhance primary care. Other models made that funding more difficult to obtain.<\/p>\n\n\n\n<p>It\u2019s unclear whether LEAD will draw more enrollment than Making Care Primary, but Hathaway, who has years of experience with elements included in LEAD, is skeptical of the program.<\/p>\n\n\n\n<p>\u201cAnytime you put multiple layers of bureaucracy between us and the patients and the dollars, it just costs more,\u201d he said.<\/p>\n\n\n\n<p>Hall said doctors \u201csee the suffering firsthand\u201d but often feel helpless in a system that cuts people off from primary care. She has been frustrated with the nation\u2019s health care system, she said, which she feels focuses on profits over patients.<\/p>\n\n\n\n<p>\u201cWe should be rolling out the red carpet for everybody to come see their family doctor and keep them out of the emergency room,\u201d Hall said. \u201cThat would keep costs lower for the whole system.
