They felt like they had no one to confide in \u2014 not their foster parents, not church leaders, not their caseworker.<\/p>\n\n\n\n<p>\u201cTo my knowledge, I don\u2019t remember ever taking a suicide screening,\u201d Hinkle said. \u201cNo one ever said: \u2018Are you having thoughts of taking your life? Do you feel hopeless?\u2019\u201d<\/p>\n\n\n\n<p>With their psychological and behavioral health needs left unaddressed, Hinkle\u2019s depression and suicidal thoughts worsened.<\/p>\n\n\n\n<p>\u201cDo I stay in the closet and feel terrible and want to end my life?\u201d Hinkle said. \u201cOr do I come out and lose all my supports, which also feels dangerous?\u201d<\/p>\n\n\n\n<p>Children in foster care are significantly more likely to have mental health issues, researchers say. And now, efforts underway to provide widespread screening, diagnosis, and treatment are threatened by sweeping funding cuts the Trump administration is using to reshape health care programs nationwide.<\/p>\n\n\n\n<p>In June, federal officials announced they would <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-news\/trump-administration-shut-lgbtq-youth-suicide-hotline-rcna213815\">shut down a suicide hotline<\/a> serving LGBTQ+ youths as part of those cuts.<\/p>\n\n\n\n<p>Children in foster care use a disproportionate amount of Medicaid-funded mental health services. Meanwhile, President Donald Trump\u2019s massive budget package, passed this month by Congress, contains substantial <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/allocating-cbos-estimates-of-federal-medicaid-spending-reductions-and-enrollment-loss-across-the-states\/\">shifts in Medicaid funding and policies<\/a> that are projected to drastically reduce services in many states.<\/p>\n\n\n\n<p>\u201cI think anybody who cares about kids\u2019 well-being and mental health is concerned about the possibility of reduced Medicaid funding,\u201d said Cynthia Ewell Foster, a child psychologist and clinical associate professor in the University of Michigan psychiatry department. \u201cThe most vulnerable children, including those in foster care, are already having trouble getting the services they need.\u201d<\/p>\n\n\n\n<p>A lack of federal standards and other system-level issues create barriers to psychological and behavioral care in the child welfare system, said Colleen Katz, a professor at Hunter College\u2019s Silberman School of Social Work in New York.<\/p>\n\n\n\n<p>\u201cWhen you\u2019re talking about anyone getting screened for suicide ideation upon entrance into the system, it\u2019s inconsistent at best,\u201d she said. <\/p>\n\n\n<div\n\tclass=\"wp-block block--newsletter  \"\n\tdata-type=\"kaiser-health-news\/newsletter\"\n\tdata-align=\"center\"\n\tstyle=\"\"\n>\n\t<h4 class=\"newsletter__title\">\n\t\t<a href=\"\/email\/\">\n\t\t\tEmail Sign-Up\t\t<\/a>\n\t<\/h4>\n\t<p class=\"newsletter__description\">\n\t\tSubscribe to KFF Health News&#039; free Morning Briefing.\t<\/p>\n\t\n<form\n\taction=\"\/email-signup\/\"\n\tclass=\"kaiser-hubspot-mini-signup-form\"\n\tmethod=\"post\"\n>\n\t<input type=\"hidden\" id=\"kaiser_hubspot_nonce\" name=\"kaiser_hubspot_nonce\" value=\"dc9818bd64\" \/><input type=\"hidden\" name=\"_wp_http_referer\" value=\"\/wp-json\/wp\/v2\/article\/2056923\" \/>\t\t\t\t<label\n\t\tclass=\"kaiser-hubspot-mini-signup-form__label--email\"\n\t\tfor=\"kaiser-hubspot-mini-signup-form-email\"\n\t>\n\t\t<span class=\"screen-reader-text\">\n\t\t\tYour Email Address\t\t<\/span>\n\t\t<input\n\t\t\tautocomplete=\"email\"\n\t\t\tclass=\"kaiser-hubspot-mini-signup-form__input--email\"\n\t\t\tid=\"kaiser-hubspot-mini-signup-form-email\"\n\t\t\tname=\"kaiser_hubspot_email\"\n\t\t\ttype=\"email\"\n\t\t\t\t\t\t\tplaceholder=\"Your email address\"\n\t\t\t\t\t\/>\n\t<\/label>\n\t<button class=\"kaiser-hubspot-mini-signup-form__submit\">\n\t\tSign Up\t<\/button>\n<\/form>\n<\/div>\n\n\n\n<p>Katz said all children entering foster care should have a brief, standardized suicide screening embedded into their initial medical assessment. And more screenings need to be conducted throughout a foster care stay, she said, because youths getting ready to transition out of the system are also vulnerable.&nbsp;<\/p>\n\n\n\n<p>Hinkle, now 31, said the summer before they aged out of the system was \u201cone of the darkest periods, because I was coming to terms with the church not wanting me to be gay and I was about to lose stable housing and whatever foster care support there was.\u201d<\/p>\n\n\n\n<p>Katz studied transition-age youths in foster care in California, which has the highest numbers of placements in foster care nationwide. According to <a href=\"https:\/\/www.jahonline.org\/article\/S1054-139X(24)00286-6\/fulltext\">her analysis<\/a>, 42% of study participants had thoughts of taking their life and 24% had attempted suicide, and she expects findings would be similar in other states.<\/p>\n\n\n\n<p>Katz also <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s10560-023-00913-4\">examined suicide screening tools<\/a> and found many that already exist could work and be easily administered by trained child welfare workers or alternative frontline service providers, or embedded in existing mental health services.<\/p>\n\n\n\n<p>Still, the quality of services varies by state and locality and can hinder attempts to curb suicides.<\/p>\n\n\n\n<p>Julie Collins, vice president of practice excellence at the Child Welfare League of America, which advocates for improvements to the child welfare system, said the gap in suicide prevention in foster care mirrors the overall nationwide void of behavioral health services for children and adolescents. \u201cThe preparation of people coming into the field isn\u2019t what it needs to be,\u201d Collins said of the lack of training for caseworkers.<\/p>\n\n\n\n<p>Ewell Foster is trying to change that.<\/p>\n\n\n\n<p>She worked with the state of Michigan to redefine and update the competencies required to earn an undergraduate certificate in child welfare in the state. Eighteen colleges and universities that offer certificate programs in child welfare in Michigan now teach about suicide prevention.<\/p>\n\n\n\n<p>\u201cIt\u2019s something the workforce has asked for,\u201d Ewell Foster said. \u201cThey need real clear guidance on what to do when they are worried about someone.\u201d<\/p>\n\n\n\n<p>So far, Ewell Foster\u2019s effort to change the wider system has not run into any roadblocks. Her work with Michigan\u2019s child welfare agency is still being funded under a grant administered by&nbsp;the Substance Abuse and Mental Health Services Administration.<\/p>\n\n\n\n<p>Agency spokesperson Danielle Bennett said such grants will continue for up to three years.<\/p>\n\n\n\n<p>However, the future of the federal agency has been in question for months. The Trump administration has laid off hundreds of its employees and has proposed folding its functions into another agency.<\/p>\n\n\n\n<p>Some states have made changes to address the foster care gaps on their own, but often it has taken legal action to spark changes in suicide prevention efforts.<\/p>\n\n\n\n<p>In Kansas, officials made several changes after the state settled the <em>McIntyre v. She had several counties agree to the project, but not enough to support her application \u2014 the study wouldn\u2019t have had enough participants to work statistically, she said.<\/p>\n\n\n\n<p>Without such studies, foster care systems nationwide can\u2019t meet the needs of children, she said.<\/p>\n\n\n\n<p>April Miller, 27, entered the system in Minnesota at age 3.
